Dose log
Record each preparation and what changed, in the shape the founder recommends for a case history. Entries are kept only in this browser, on this device.
The log needs JavaScript and local storage. Without them, the same fields work on paper.

What to write
From the founder’s guidance on case histories: before the first entry, list the starting condition in your own words, every complaint, with any medical diagnoses and test results. At each entry, record the volume, the form of preparation and the interval, and go through that list noting how each item has changed. Write facts, not interpretations, and include anything else that changed in the same period (medication, diet, events), so that you can judge honestly later. A case history, how and why to write it.
